Green Performance Strategies Delivers First Success in the Healthcare Community

The Road to Health: Community Health Profile 2012We are very pleased to announce that Green Performance Strategies has delivered its first successful project. GPS worked with the Dorothy Rider Pool Health Care Trust and the Health Care Council of the Lehigh Valley to release its first report, The Road to Health: Community Health Profile, the first step in the development of a comprehensive Community Health Needs Assessment.  The Community Health Care Needs Assessment Profile will facilitate the development of a 5-year Community Health Action Plan, which is required of healthcare systems under the Affordable Care Act.

This report  looks at the six major causes of death and illness in the Lehigh Valley – heart disease, cancer, stroke, lung disease, injuries and violence, and complications related to diabetes – and the major factors that contribute to these health issues.  The data-driven profile lays out the key indicators for health in the Lehigh Valley, including a comparison with state and national data and goals. The profile also includes a dashboard to make it easier to communicate results to the community.
